An application can be found here and may be emailed or mailed. Please look at the requirements and guidance for the eligible practice you are interested in before applying!

Eligibility
- 75/25 cost-share rate not to exceed $200.00 (Reimbursed 75% of expenses, up to $200)
- One application per household (Warren Co. residents only)
- Funding is first come, first served
- Application must be approved before starting the project
- Project must be completed within 90 days of the respective project guidelines
- Photographs and receipts must be submitted within 90 days for reimbursement
